For chronic ankle instability, a 2024 review of nine randomised trials and 341 patients found balance training improved FAAM Sports scores by 17.74 points against control, and matched strength training on dynamic stability while scoring ahead of it on function. Ankle work is the one use of a balance board with a dedicated body of trials behind it, and it is also the use with the clearest rule about where to start. This page sets out what the research reports, what proprioception training involves in practice, and where a 43.5 by 33 cm single-axis rocker board sits among the objects sold for ankles. Nobody here has examined your ankle: if the joint still gives way, or the injury is recent, the plan comes from the clinician treating you.

What the trials report on ankle instability

Nine randomised trials covering 341 patients with chronic ankle instability were pooled in 2024. Balance training improved the FAAM Sports score by 17.74 points against control and the CAIT score by 3.95 points. Against strength training it was equivalent on dynamic stability and ahead on the functional scales.

Two scores carry that result. CAIT, the Cumberland Ankle Instability Tool, asks how often the joint gives way. FAAM Sports is the sports subscale of the Foot and Ankle Ability Measure: running, cutting and landing rather than walking across a room. Both are reported by the patient, so they measure what people complain about after a sprain, not a laboratory proxy.

The comparison with strength training is worth reading twice. On dynamic postural stability the two were equivalent; on the functional scales, balance training came out ahead. That is the argument for training balance as its own thing rather than assuming it arrives with stronger calves.

That review cannot tell you whether your own ankle is ready for any of it. It enrolled people with chronic instability — a joint that has healed and still gives way — not ankles inside the window after a fresh sprain. If yours is swollen, painful or recent, the starting point is set by the person treating you.

What proprioception training actually is

Proprioception is the sense of where a joint sits without looking at it. Training it means putting the joint into positions it has to correct, over and over, until the correction happens before you think about it. A tilting surface makes that correction continuous instead of occasional.

On a firm floor the ankle corrects all day and the rest of the body ignores it. Put the same foot on a surface that tips and those corrections become the task. That is why the clinical literature names this exercise by what it demands — balance and functional training — rather than by the apparatus.

It is also why the order of the steps decides whether a session trains anything. Two feet is easier than one, eyes open easier than eyes closed, a hand on a worktop easier than none. Our ten-step progression is built on that order.

The wider evidence, and what it is rated

Balance and functional exercise reduces the rate of falls by 24%: rate ratio 0.76, 95% CI 0.70 to 0.81, from 39 studies and 7,920 participants, rated high-certainty evidence. Strength training measured on its own in the same review showed no demonstrated effect, at a rate ratio of 1.14 with an interval running from 0.67 to 1.97.

Those figures come from the 2019 Cochrane review, which pooled 108 randomised trials and 23,407 participants. It classified trials by exercise type rather than by equipment, which is why no board, disc or cushion appears in its conclusions.

The same review reports a 56% reduction in fractures, a risk ratio of 0.44 from seven trials. Hold that one loosely: the interval is wide and seven trials is a thin base. A number quoted without its interval and its certainty rating is decoration, not evidence.

A benchmark for the ankle you are training

Stand on one leg on the floor and time it, each side. Pooled across 22 studies and 3,484 people, mean single-leg stance times were 27.0 seconds at 60–69, 17.2 seconds at 70–79 and 8.5 seconds at 80–89. On a tilting surface, expect a fraction of that at first.

Ankle training has no visible output: nothing gets bigger, and three weeks in you cannot tell whether anything changed. Timed on the floor once a fortnight, each leg, you get a number that moves — and the gap between the two legs says more than either alone.

Dedicated ankle boards are a different object

A dedicated ankle board is small, takes one foot, and usually tips in every direction on a dome or a half-sphere. It is sized for a clinic bay and for single-joint work. A rocker board is a larger deck on two curved rails that tips one way and back, and holds two feet.

Two objects, two jobs. The single-foot disc is sized for one joint at a time in a clinic bay. The rocker board gives a deck long enough for two feet, which is where every progression starts and where the two-footed holds are done.

Slant boards: the same joint, the opposite mechanism

A slant board is a fixed or adjustable wedge. You stand on it and it holds one angle, loading the calf and the Achilles as a sustained stretch. A rocker board does the opposite: the angle never settles, and the work is in correcting it. One trains length under load, the other trains control.

The two get shelved together because both sit under the foot and both come up in ankle programmes, but they ask for different things. On a wedge, the ankle is held at end range and the soleus and gastrocnemius lengthen against body weight — which is why the search terms around slant boards cluster on calf stretching, Achilles work and squat depth rather than on balance. Heel elevation is the same idea applied to a lift: raise the heels a few degrees and the knee can travel further forward, so a squat reaches depth without the ankle running out of dorsiflexion.

Adjustable models change the angle with wedges or a hinge, commonly through a range of about thirty degrees; fixed ones hold a single angle and cost less. Neither moves under you, which is the whole point: a stretch you have to fight for is not a stretch. A rocker board used for the same purpose would give you a moving target and no held position.

Deck size against foot length

The deck is 43.5 cm long and 33 cm across. A EU size 46 foot runs about 30 centimetres, which leaves roughly a finger’s width of board at the toe and at the heel. Two of the three longest written reviews say the same thing: the surface is too small for a EU size 46.

Standing across the rails, your foot lies along the 33 cm dimension, so foot length decides the fit — not the 43.5 cm figure quoted first. European sizes step in units of about 6.7 mm of foot length, so each size down puts roughly 3 mm more board at the toe and 3 mm at the heel. At EU 46 the foot covers nearly the whole width: workable for two-foot holds, demanding for single-leg work.

Where a board fits in an ankle programme

After the clearance, not before it. Floor before board, two feet before one, wall within reach, shoes on. Short sessions on most days beat one long session a week: the programmes that worked in the Cochrane review ran twelve weeks or more, nearly a third a year or more.
  • Clearance first. A joint that still gives way, a sprain inside its healing window, or pain nobody has looked at are questions for a clinician. The 2024 review enrolled patients already assessed.
  • Both ankles, every session. The uninjured side is the benchmark that matters: same body, same day, same shoes.
  • Shoes on. The deck carries a grit anti-slip surface and is used in footwear, not barefoot.
  • Something within arm’s reach. A worktop or a doorframe. Not a medical point, a practical one: the first three weeks are harder in an empty room.
  • Stop when control goes. Fatigue takes coordination before it takes strength, and the last two minutes of a long session are where people come off.
  • Remove support, not time. Thirty seconds of a controlled hold with no hands beats three minutes hanging onto a worktop.
